Breast Tumor Antigen (Ca15-3)

A high molecular weight, 300-450 Kd, mucin-like glycoprotein. Localized in the alveoli and ducts of mammary glands, the antigen also exists as circulating antigen. Distinctly different epitopes are identified by monoclonal antibodies DF3 and 115D8. Other mucin cancer assays such as Ca27.29 identify slightly different epitopes all are expressed variably on the same set of tumor related mucins. Ca15-3 levels are elevated in most patients with advanced breast cancer making the major clinical role of this marker to be an aid in the early detection and relapse of breast cancer. In combination with Ca125, Ca15-3 is also useful in early detection and relapse of ovarian cancer. Increased Ca15-3 levels are also associated with colon, lung, and hepatic tumors.

Gastro Intestinal Tumor Antigen (Ca19-9)

A glycoprotein of 210Kd, Ca19-9 is a carcinoma cell surface antigen whose carbohydrate determinant is defined as a sialylated lacto-N-fucopentaose II, a sialylated Lewis A blood group antigen. Elevated in most patients with pancreatic cancer, Ca19-9 is also variably (20-50%) elevated in patients with colorectal, gastric and hepatocellular cancer. Together with CEA, elevated Ca19-9 is suggestive of gallbladder neoplasm in the setting of inflammatory gallbladder disease.

Ovarian Tumor Antigen (Ca125)

A 200Kd cell surface glycoprotein associated with cells of ovarian carcinoma. It is a mucin-like glycoprotein originally identified in human ovarian tumors of serous, endometroid, and clear cell types. Ca125 is indicated for use in the detection of residual ovarian carcinoma in patients who have undergone first-line therapy and should be considered for diagnostic second-look procedures. Although not FDA approved for these indications, Ca125 is elevated in patients with lung, cervical, fallopian tube, and uterine cancer and in endometriosis.

Alpha Fetoprotein Antigen (AFP)

A 70Kd major fetal serum glycoprotein, function unknown. Synthesized in liver, yolk sac and developing fetal gastrointestinal tract serum elevations are seen in pregnant women and some patients with cirrhosis and hepatitis. Increases in serum levels aid in diagnosis, classification and monitoring of non-seminomatous testicular cancer as well as primary hepatocellular carcinoma. Approved for non-seminomatous testicular cancer and neural tube defects in the U.S.

Carcinoembryonic Antigen (CEA)

A family of 200 Kd glycoproteins consisting of a single polypeptide chain with varying carbohydrate components. Elevated CEA levels may be detected in smokers, patients with colorectal polyps, pancreatitis, liver disease, pulmonary infections, inflammatory bowel disease and renal failure. The greatest clinical utility of CEA is as a warning sign of tumor recurrence post-operatively after resection of colon cancer.

Tag72-4 Antigen (CA72-4)

A high molecular weight, mucinous glycoprotein originally defined by monoclonal antibody B72.3, TAG-72-4 has been found widely distributed in different types of carcinomas. Potential immunohistochemical uses include aiding in the classification of epithelial-derived cancers and in the differentiation of adenocarcinoma from mesothelioma. TAG72-4 is useful as a follow-up look of gastric cancer patients under treatment or in remission.

Cytokeratin-19 Antigen (Cyfra-21-1)

A soluble, 40kd cytokeratin fragment found in the cytoplasm of epithelial cells that is released into the serum upon cell death. Clinical studies of lung and bronchial cancer patients, in particular, have shown Cyfra-21-1 to be useful in diagnosis and follow up of non-small cell and squamous cell carcinoma of the lung.

Tissue Polypeptide Antigen (TPA)

A single chain polypeptide that can be resolved into components of 20Kd and 45Kd. TPA has been reported to be related to the cytoplasmic intermediate filaments, specifically cytokeratins 8, 18 and 19, and is found in the cytoplasm of epithelial cells. TPA is elevated in gastrointestinal, urological and gynecologic cancers.

Ferritin Antigen

Ferritins (MW:~450Kd) are polymeric proteins that reversibly bind and store iron in liver, spleen and bone marrow. Clinically, the monitoring of serum ferritin levels aides in the diagnosis and management of iron metabolic disorders. Serum levels increase in hepatic diseases, various malignancies, inflammation and late stage hemochromatosis. Ferritin levels are also indicative of the erosion of iron stores during pregnancy.
